The certification for the Professional Supervisor of the Audiometric Component of the Hearing Conservation program is valid for five years. The class of 2007 will need to re certify in 2012. The following criterion has been outlined for recertification.

Option 1

• Successfully complete the online Professional Supervisor (CPS/A) exam given in 2012.
  ****Successful completion means scoring and 80% or higher on the exam.
• Submit the re certification fee of $125.00

or

November 9, 2012 - December 10, 2012
Registration deadline: Friday, November 1, 2012

Option 2

• Register and attend one of the workshops being offered in 2012
  • November 8, 2012 - Phoenix, AZ
    Pre conference workshop to the American Doctors of Audiology Annual Conference
• Successful completion of the online Professional Supervisor (CPS/A) exam given after each workshop.
  Successful completion means scoring and 80% or higher on the exam.
• Submit the appropriate workshop registration and exam fee.
